Figured it out, the new update made Cloud Save Required. If you don't have steam's "Save to Cloud" enabled, Iron mode won't work. Not sure if intentional or not. :x

I think the update added more checks to achievements.

I used the updated table, and it says it can earn achievement when I turned on Iron Man.

But I am not getting any achievements.

I think it is a bug with the game itself. Even with no mods on and not using cheat engine, when I enable Iron Man, once game session starts, it becomes normal and not iron man session (still has console and save and load features).
There does indeed seems to be another check. The first one seems to work when you create a game, but if you exit and load, it didn't. Added a new check that hooks into the game loop, but I am not sure I am able to test it (for actually getting achievement). So if you can test the table here (I labeled it beta, as testing is.. non-existent :) )

How to use this cheat table?
  1. Install Cheat Engine
  2. Double-click the .CT file in order to open it.
  3. Click the PC icon in Cheat Engine in order to select the game process.
  4. Keep the list.
  5. Activate the trainer options by checking boxes or setting values from 0 to 1

i think they do something with the check on achievement, load save game(cloudsave) will disable it even with the table. Check save game with notepad and the line "achievement=" no longer there. Or just use the StellarisChecksumPatcher provide by steam forum to bypass the mod achievement thing while wait for the table to be fix

So I tested it, achievements work starting up a game and playing with it but if you save and exit (I have cloud saves enabled as well if that matters) and go back in to the game I was not getting achievements. I had the debug check thing checked and it was saying it had the correct values. I tested it by trying for the "Burning Brightly" achievement which is build a dyson swarm and arc furnace in the same system and it did not pop even after waiting for many years and yes confirming both are fully upgraded.

Of course I download this last night, and then Stellaris updates to 3.14.1592 (3d78) this morning 💀 Seems to be broken now unfortunately. "The array of byte named MSHL could not be found."

Never mind, It kept crashing because I was using Cheat Engine 7.5 when I should have been using 7.4.

i have the most resent table v3.14.1592 (3d78) but the extension v3.14.15 extension dosent seem to work the questiopn is why :D
