The greatest interest here is the items from the store.
How to use this cheat table?
Install Cheat Engine
Double-click the .CT file in order to open it.
Click the PC icon in Cheat Engine in order to select the game process.
Keep the list.
Activate the trainer options by checking boxes or setting values from 0 to 1

The greatest interest here is the items from the store.
There are people like me who just want to edit the base-game armors for their looks and perks, so there's definitely no harm if we can get as many hash IDs as possible.
Also, Ubisoft probably won't just let it slide if people actually manage to mod store items into their saves without buying them.

Good job guys for finding out the addresses and working so fast on an eventual gear swap!
@budabum: I tried to mess around with your quick tutorial and I seem to be confused with this very step:
