Steam version Table for the static addresses for most of the non dummy items.

Only thing not finished is the Ranged weapons for Yusuke, Makoto, Haru, Akechi, and Kasumi as the game doesn't have anyway to check the Ranged Weapon list unless you have the character in your party.

Update

Updated 2: Added Names to all item addresses, removed more non working or game crashing items lines, and cleaned up Essential Items into as many separate categories as possible

Cleaned out more dummy item addresses and separated out the Items part into multiple categories

Also updated the Ranged Weapons addresses
