Is this safe to use or you can risk a ban from Microsoft?

It's absolutely safe, I have used it on 50+ games and there's no ban yet. Also, it doesnt work on all the games, specially games that has event based achievements.

However, it does work on Death Stranding so enjoy

if you care about your Xbox account which is also your Microsoft account, don't use it.
Xbox is not Steam, they care about achievements and gamerscores.

it's Not safe at all. it's against Xbox's terms of service.

using Cheat Engine tables to unlock achievements never got anyone banned as far as I know, but that tool is dangerous and can get you banned at any moment.
