IDK how old the game is now, but here's a table from 2013 for CZ.

EDIT: And Black Mesa but it uses AoB scan should work. 2015.

I eventually just switched to playing HL2 and HL1 games inside of Garry's Mod with Lua scripts. [Link]

EDIT2: I found a table for Garry's Mod. It allows changelevel2 to be called between any map in MP, but your creations won't carry over, you gotta use the duplicator.

EDIT3: Here's opposing force, I think this is from 2014, unknown build. My notes are non-existent.
