I just checked and tried getting the "watch your step" achievement for being hit by a train and dying, didn't have it on steam yet. Had steam-achievements enabled on the table whilst using a mod on my game. Table doesn't prevent it disabling steam-achievements.

Yes, I activated that so that I could activate the steam-achievements script, both were enabled at the time of the achievement firing.
In-game the achievement registers, but it doesn't activate a steam achievement for it.

Well that's weird, I'll create a "Debug Build" which may help find the issue.

It basicaly means that the bytes "48 C7 44 24 20 FE FF FF FF" don't match up with the bytes at address "Factorio.Player::getUsernameWithTag+6".
But I have actually checked and they seemed to be the same for me at least, so have you tried restarting your game?
If it still doesn't work, then just write another comment and I'll find another Injection spot.

It worked well when I first used it
At some point it suddenly doesn't work.
