I just use Current Selected Item script to get the item string id, then I go to the weapon I want to modify and change the id.
Also, I just noticed I made a mistake, if anyone is using the npc equipment hack I posted before, only use it to change "Clothes(Default)" and "Accessory" values.
The rest of the addresses are most likely off because the order can be different for everyone.

Basically, it's like this, imagine each npc character has his own inventory(slot 1 to slot 200 for example), let's say Kirito's starting inventory look like this:

Slot 1: Nocturnal Cloak M [Equipped][Default]
Slot 2: Assassin Jacket M
Slot 3: Promise Ring
Slot 4: Kotetsu G4++ [Equipped Weapon 1]
Slot 5: Kotetsu G4++ [Equipped Weapon 2]
Slot 6 onwards: Nothing

Now this item ordering is most likely the same for everyone because this is Kirito's starting equipment, but this gets more complicated as he levels up, because apparently, the game automatically gives him(and other npc) new equipment after reaching certain levels.
So let's say the item in his Slot 4 and 5 is what he uses until level 30, once he reaches level 31, the game gives him 2 new Kotetsu G4++ (automatically equipped),
so now his inventory looks like:

Slot 2: Assassin Jacket M
Slot 3: Promise Ring
Slot 4: Kotetsu G4++
Slot 5: Kotetsu G4++

Slot 7: Kotetsu G4++ [Equipped Weapon 2]
Slot 8: Nothing

Let's assume at this point you've unlocked Kirito mode and then you proceed to change his clothes to Battle Jacket M for example, his inventory now looks like:

Slot 4: Kotetsu G4++
Slot 5: Kotetsu G4++

Slot 8: Battle Jacket M [Equipped]

See in this save file you give Kirito new clothes once he reaches level 31, so his new clothes is now in Slot 8, but if in other save you give him new clothes at level 70, his Battle Jacket M will be further down after slot 8 because slot 8 will be occupied by his new Kotetsu.
