I taken a look at it, and I think MA has done something differently in the past couple of updates. I found the address that manipulates the number of gear perks you have, but I think the address point to the same address as the one in the table, both having the same results.

I think there might be a hard gear perk slot limit elsewhere. Other addresses I found don't do anything.

Initially I thought this was a visual bug, being unable to see the fourth perk, but the perk you assigned, does not work in-game too.
a table maker could posibly fix this.
